[Dhsbb-jazz] permission slips

[DHS Band Boosters - Jazz Band]

Dear Jazz Band Parents,

Permission slips for the CSUS Winter Jazz Festival on 12/02/06 were distributed to the students. Please complete the forms and have your child return them on Monday, November 27 (if not done so already).

As Lesli pointed out, if you plan to provide transportation to kids other than your own, you'll need to complete forms 3a-b AND submit a copy of your car insurance card.
