After filling out the form, it wants to send you a reply to let you know you
have successfully completed the form. But there is no place for an email
except in the chaperone section.</DIV>

<DIV><STRONG>Please enter: Chaperone 1 email, so the form can reply to you
that you have submitted it. (If you don't want to chaperone don't fill out
anything else in the chaperone section).</STRONG></DIV>

<DIV>Or, you can also simply ignore the message that says your form wasn't
submitted due to lack of email....the forms seem to be getting through
anyhow. </DIV>
